This form will be sent to the Integrated ICT Service Desk.

During the off-boarding process, we will remove: 

• our monitoring and automation agents 
• remote control agent 
• our endpoint antivirus protection if supplied by Integrated ICT 

Once the device is marked as off-boarded or retired, it will no longer be supported by Integrated ICT and any further troubleshooting/change requests on this device will be charged. If the machine is to be disposed of, it is the responsibility of the client to dispose of the machine. Integrated ICT offer solutions such as secure hard drive erasure and e-waste recycling solutions.
