Please complete all fields in this form so we can set up their user account correctly. Once submitted, the form will auto send to the Integrated ICT Service Desk which will create a service ticket and be assigned to the next available technician. We’ll keep you updated on the progress of the service ticket via email, and if needed, we’ll also give you a call once the setup is complete. Please contact us with any queries.
